Other Incidents/Issues


Description:


Any incident that impacts or might impact the safety of employees or customers or the operation of a Liberty program that does not fall within the other incident descriptions. Incidents/Issues that Require Reporting Directly to Cients/Programs: Some of Liberty's clients or programs such as CA DDS require that certain types of incidents or issues be reported directly to them, in addition to reporting them to Liberty. If a client or program requests such reporting, we must comply with it.
 

Reporting Instructions:

  1. Notify the program director or manager immediately by phone, email or in person of any potential safety risk or incident.
  2. Complete the Incident Reporting Form within 24 hours and send via secured Liberty or encrypted email to:
  3.  The Program Manager/Director, in consultation with the VP of Operations responsible for the program, will direct any follow-up related to this incident, and will notify/consult with appropriate corporate administrators as needed.
