Types of Reportable Incidents

Sentinel Events
Any unanticipated event occurring in a healthcare setting resulting in death or serious physical or psychological injury to a patient or patients, not related to the natural course of the patient’s illness

Corporate Compliance Incidents/Issues
Any suspected violations of laws or regulations, Liberty policy or procedure, or Liberty contract by Liberty employees, contractors, clients or vendors

HIPAA Privacy Incidents/Issues
Improper disclosure or use of protected health information

Employee Accidents and Injuries
Any incident that results in harm to the employee, even minor injuries, that occurs while the employee is on duty

Violations of Professional and Personal Boundaries
Inappropriate conduct by Liberty employees, contractors or vendors that violates professional ethics or boundaries of professional/client relationship

Legal Matters/Issues
Situations that may reasonably lead to or relate to a legal claim against Liberty or any of Liberty’s employees or subcontractors, including receipt of subpoena or notice to appear at a hearing, or requiring response on behalf of Liberty.

Incidents That Require Reporting Directly to Clients/Programs

Other Incidents/Issues
Any incident that impacts or might impact the safety of employees or customers or the operation of a Liberty program that does not fall within the other incident descriptions.
