Description:
These incidents refer to situations that:
- may reasonably lead to or relate to a claim
against Liberty or any of Liberty’s employees or subcontractors;
- involve receipt of a subpoena or notice to appear
at a deposition or hearing as a representative of
Liberty;
- involve receipt of a legal document that
requires a response on behalf of Liberty.
It is extremely
important to complete and submit the Incident Reporting Form and consult with Liberty's
Legal Department prior to responding to any of the above
incidents so that they can advise the employees or subcontractors
involved and manage the incident to mitigate risk.
Reporting Instructions:
- Complete the
Sentinel Event Form within 24 hours and
send the report via Liberty secured or encrypted
email to the following:
- The Liberty Legal
Department will direct any follow-up needed to resolve the incident.
- Incidents which are
reported through Liberty’s sentinel event reporting
and investigation process will be forwarded to the
Legal Department by the VP of QPQI if it is determined
the incident may give rise to a claim.
