Incidents Reporting Clients Programs


Description:


There are incidents where Liberty's contracted client or program requires Liberty to report certain incidents directly them, in addition reporting them to Liberty.
 

Reporting Instructions:

  1. Report these incidents to the applicable client or program in the manner and time that is required by the client/program.
  2. If the incident is also a type of incident that is required to be reported to Liberty (from one of the categories above such as Corporate Compliance or HIPAA/Privacy), please also report it in accordance with the applicable reporting requirements.
  3. If you are not sure what to do, please contact your supervisor or the Chief Compliance/Privacy Officer - John Beck.
