HIPAA/Privacy and Security Compliance Issues


Description:


HIPAA data incident refers to the improper disclosure or use of protected health information (see HIPAA Privacy and Security Plan). Anyone who knows or has reason to believe that an individual's privacy or confidentiality has been violated, or another person has violated Liberty’s HIPAA Privacy and Security Program shall report the matter promptly.

 

Reporting Instructions:

  1. Anyone who knows or has reason to believe that an individual's privacy or confidentiality has been violated, or another person has violated Liberty’s HIPAA Privacy and Security Program shall report the matter promptly to his or her supervisor OR Liberty’s Chief Compliance Officer/Privacy Officer at 610-668-8800 Extension # 405 OR call Liberty’s Employee Help Line at 1-800-653-7174.
  2.  A Sentinel Event Form (click on this link) should be completed and sent via email within 24 hours to the Liberty Chief Compliance Officer/Privacy Officer John Beck at corporatecompliance@libertyhealth.com.
  3. The Liberty Chief Compliance Officer/Privacy Officer will contact you for further follow-up.
